We are seeking a dedicated and compassionate renal healthcare professional to join our Renal Services team. This role focuses on providing education and support for patients with Chronic Kidney Disease (CKD) stages 1 to 3a, aiming to enhance health outcomes through proactive care and education.
* Deliver clinical and educational services to referred patients, ensuring high-quality patient care.
* Consult with inpatients suffering from CKD and diabetes, developing individualized care plans.
* Collaborate with key stakeholders to provide relevant information and assistance to patients and their families in decision-making processes.
* Participate in preventive kidney health promotion initiatives and work with various service providers to implement best-practice strategies.
Key Responsibilities
* Oversee the planning, implementation, and evaluation of CKD services in the region, contributing to quality care and continuous improvement efforts.
* Provide individualized, quality clinical care to patients with CKD and End-Stage Kidney Disease (ESKD).
* Facilitate appropriate clinical education and support to healthcare staff, patients, and their families.
* Actively participate in clinical discharge planning and follow-up care.
* Coordinate patient care in collaboration with other healthcare professionals, utilizing a problem-solving approach.
* Monitor and implement nursing practice standards, promoting clinical governance within the department.
