Role Overview:
Join us at UnitingSA's Community Aged Care as a Physiotherapist (PT) and make a real difference! Work closely with clients at home, crafting bespoke physio plans. Under the Program Manager's guidance, support our vibrant team in delivering impactful care tailored to individual needs. Passionate about empowering older adults? This is your opportunity to shine!
Key Responsibilities
* Conduct physiotherapy assessments, develop treatment plans, and evaluate the effectiveness of interventions.
* Collaborate with clients, families, and teams to consult and strategise.
* Tailor treatment plans to boost client independence and safety at home and in the community.
* Educate individuals and families on identified risks and preventive measures according to their needs.
* Apply evidence-based interventions during comprehensive assessments and recommendations.
* Assess community-specific aspects: showers, kitchens, and car transfers.
* Compile concise clinical reports to track progress and recommendations.
* Execute personal care safety evaluations and aid in care plan development.
* Facilitate prompt referrals to other healthcare professionals for support.
* Complete referrals to other health care professionals promptly.
* Advocate for and connect clients with valuable community resources.
Skills & Experience:
* Prior work in community aged care (HCP, TCP, CHSP) or similar healthcare settings (desirable)
* Conducted diverse physiotherapy assessments and screenings.
* Extensive experience in thorough assessments with diverse client groups.
* Developed personalised treatment plans and clear recommendations.
* Delivered evidence-based physiotherapy interventions in communities.
* Familiarity with Consumer Directed Care and relevant programs (CDC/HCP, CHSP, TCP).
* Understanding of Aged Care Quality Standards.
* Knowledgeable about the Physiotherapy Code of Ethics.
