About the Role

The role involves working across all healthcare facilities within the district, delivering multimodal care including home visits, telehealth, and community-based consultations and conducting timely patient assessments, guide clinical decisions, and support early interventions, enhancing continuity of care and reducing avoidable hospital admissions. The position also requires cross-functional involvement across Healthcare in the Home (HITH), Virtual Care Services (VCS), and on-call services.

There are opportunities for extended scope of practice, including contribution to projects and developing clinical pathways such as Palliative care in the home, Emergency Department (ED) avoidance strategies, Neurology and surgical virtual care initiatives.

This position has on-call responsibilities outside of regular hours to support timely patient care, with a minimum commitment of 1–2 weekend shifts per month.

This role reports to the Clinical Director of VCS and HITH and is integral to delivering high quality, patient-centered care across the district on weekends.

Where You'll be Working

Services are provided District-wide, therefore there is an expectation to work across multiple sites, including Northern Beaches, Ryde, Hornsby, and patients' homes, and to conduct care flexibly through home visits, clinic visits, and virtual consultations as service needs arise

Purpose of the Position

You will provide clinical management and treatment of patients under your care in a Northern Sydney Local Health District facility or service. This care will be provided within the scope of clinical practice endorsed by the NSLHD Medical and Dental Appointments Advisory Committee.

Key responsibilities include:

• Providing medical governance and clinical oversight of weekend HITH and virtual care patients

• Conducting timely assessments and reviews via multimodal consultations

• Managing complex chronic and infectious conditions in community settings

• Supporting care planning and clinical risk mitigation

• Collaborating with nursing, allied health, and virtual care teams

• Contributing to hospital avoidance and improving service responsiveness

The position will provide clinical consultation services within NSLHD and will contribute to the teaching and training of medical students, graduates and other health care professionals and to research, evaluation and quality improvement activities within NSLHD.

This position is temporary with the possibility of temporary extension based on performance, funding and need.

Selection Criteria

1. MBBS or equivalent, currently registered or eligible for registration with the Medical Board of Australia. Applicants must have Fellowship of the Royal Australasian College of Physicians (FRACP) or recognition as a specialist by the Medical Board of Australia. Applicants expecting to be awarded their Fellowship within 3 months may be considered.
2. Consistently demonstrates behaviours that reinforce the CORE Values of our organisation; Collaboration, Openness, Respect and Empowerment. Demonstrates these behaviours with all stakeholders; colleagues, direct reports, as well as our patients and consumers, and those that care for them.
3. Sound clinical knowledge in Geriatric Medicine demonstrated through successful clinical practice in an environment appropriate to the requirements of the position including evidence of participation in quality assurance activities relevant to the specialty.
4. Demonstrated clinical, diagnostic and technical skills commensurate with functioning as an independent Geriatric Medicine Consultant
5. High level interpersonal and communication skills and demonstrated ability to lead and work within a multidisciplinary team environment.
6. Demonstrated participation in and commitment to undergraduate, postgraduate, interdisciplinary teaching and education, including research, quality improvement, patient safety and risk management.
7. Preparedness to participate in any on-call service, including on-call rosters already existing or established.
8. Current NSW Drivers licence.
