About The Role
In this role, you will be responsible for providing Care Management services to a caseload of Home Care Package clients. You will develop and review care plans, oversee the allocation of funds for clients and their carers, and ensure high quality service delivery within the Home Care Package framework.

Essential Experience
Preferred Candidates
Preferred candidates will be required to undergo a National Police Check as part of the recruitment process (funded by the City).
