Social Worker – AHP1/AHP2 (Northern Adelaide Local Health Network – Lyell McEwin and Modbury Hospitals)
Salary: $73,710 – $110,052 per annum plus superannuation and salary sacrifice benefits
About the Role
Provide essential social work services in a dynamic acute hospital environment. Work with multidisciplinary teams to support patients and families through challenging moments and complex discharge planning.
Key Responsibilities
- Conduct comprehensive bio‐psychosocial assessments to inform care planning and discharge decisions.
- Provide brief crisis counselling, advocacy, and direct support to patients and carers across acute and sub‐acute settings.
- Coordinate complex discharges in collaboration with consumers, carers, medical, nursing and community‐based services.
- Contribute to culturally person‐centred practice and safe practice, particularly for Aboriginal and Torres Strait Islander patients.
Qualifications and Experience
- Recognised finalised qualification in Social Work and eligibility for AASW membership.
- Current eligibility to work in Australia.
- AHP2 level with a minimum of three years hospital experience, including supervising staff.
- Ability to build confidence in professional practice, navigate complex discharge planning, and communicate clearly in verbal and written form.
- Prepared to discuss management of Domestic Family Violence, child/elder protection, grief and loss, and relevant legislation (e.g., Mental Health Act, Guardianship frameworks).
- Capacity to work within a 7‐day roster, including weekend and public holiday shifts, and to manage competing clinical priorities under stressful circumstances.
