Role Description
Bundaleer is seeking an experienced Clinical Case Manager to provide high-quality clinical oversight and governance for clients with clinical care needs in a community-focused organisation that empowers older Australians to live with dignity, choice, and independence.
This full-time role will involve overseeing clinical assessments, complex care planning, and clinical governance within the home care environment. You will provide expert guidance to non-clinical Case Managers and home care workers, ensuring clinical risks are identified, managed, and mitigated.
Key Responsibilities
* Conduct comprehensive clinical assessments and develop client-centred care plans
* Lead the clinical governance aspects of home care, including incident management, risk assessment, and mandatory reporting
* Provide clinical mentoring, education, and competency assessments for home care staff
* Oversee delivery of complex clinical care in accordance with scope of practice and evidence-based guidelines
* Ensure all care aligns with Aged Care Quality Standards, including clinical governance (Standard 8)
* Support investigations of clinical incidents, complaints, and hazards
Requirements
You will be a registered nurse with a strong understanding of Home Care Packages, NDIS, DVA or other clinical care funding streams. Your skills and qualifications include:
* AHPRA registration as a Registered Nurse
* Minimum one year of experience as a Clinical Case Manager in home care
* High-level knowledge of clinical governance, risk management, and evidence-based practice
* Commitment to consumer-directed care and shared decision-making
* Current NDIS Worker Screening Check
