About us:
The CQHHS GERI/RaSS is an Emergency Department (ED) substitution model and is designed to support hospital avoidance and the frail older person through their hospital journey. The RaSS component of the service is supported by a telephone triage service providing clinical advice and staff deployment options to RACFs' offering Emergency Department substitution services to acutely unwell Residential Aged Care Facility residents.
The GERI component of the service is a multidisciplinary team based in the ED and is responsible for streamlining, and where possible, fast tracking frail elderly patients through the ED by active management of patient referrals, comprehensive assessment and referral to community-based services. The GERI/RaSS service also provides an in-reach service to support early discharge of residential aged care facility residents back to their home.
About the role:
The Clinical Nurse (CN) performs an early leadership role inside the career pathway for nursing within CQHHS and can function in more complex situations while providing support and direction to nursing personnel integrating theory, practice and experience while providing support and direction to nursing staff and enhancing the quality of Geriatric nursing care. The CN follows defined service quality standards, occupational health and safety policies and procedures relating to the work being undertaken to ensure high quality, safe, evidence-based practice within services and workplaces.
About you:
* Current registration through the Australian Health Practitioner Regulation Agency (AHPRA).
* A relevant postgraduate certificate or post graduate diploma is an expectation of this position. Where the successful applicant does not currently hold this qualification, commitment to completion within an agreed timeframe is expected and will form part of the performance and development plan.
What we offer:
Total remuneration value of up to $114,045 p.a. is comprised of:
* An hourly rate between $ $60.05 p.h.
