Chronic Disease Care Coordination Role
We are seeking a passionate Registered Nurse to support clients with chronic conditions through culturally safe, holistic care coordination. The successful candidate will work closely with healthcare professionals to assist with chronic disease management plans, health checks, and referrals to allied health and community programs.
* This role involves coordinating care across internal teams and external providers, supporting medication management, and contributing to case management for complex clients in partnership with the Chronic Disease and Outreach teams.
Responsibilities:
* Develop and manage chronic disease care plans
* Run effective chronic disease clinics
* Deliver preventative health care, health promotion, clinical support, and minor procedures
* Work collaboratively with a multidisciplinary team
Requirements:
* Current APHRA registration as a Registered Nurse (Division 1)
* Experience working in chronic disease management
* Demonstrated experience in delivering preventative health care
* Understanding of health issues impacting Aboriginal and Torres Strait Islander communities
* Confidence using Microsoft Office and electronic client management systems
* Ability to work autonomously and take initiative
