About the Position
This role requires a skilled practitioner to work with families in partnership, fostering strong relationships and empowering them to build their agency. As a Family Support Practitioner, you will deliver interventions that build family awareness and insight into their history and current challenges.
Responsibilities
* Meet individual case performance targets
* Provide high-quality practice in all interactions with clients
* Facilitate client-led assessments and Care Plans
Requirements
To be successful in this role, you will require a current bachelor's degree or diploma in Social Work, Human Services, or related field, and 2-3 years' relevant experience in social services, including case management.
