About The Role
The Nurse Practitioner (NP) or Transitional Nurse Practitioner (TNP) will enhance access to healthcare for older persons by addressing service gaps within dynamic Models of Care (MoC).
Responsibilities include improving health outcomes for older persons by providing timely access to specialist aged healthcare, preventing deterioration, reducing hospital presentations and admissions, while collaborating with other healthcare team members.
The primary metaspecialty is 'Aged Care', with supported development of your Scope of Practice (ScOP) into 'Emergency and Acute Care', 'Mental Health', and 'Chronic and Complex Care'. Departmental locations within the expanding MoC may include: Inpatient Units, Outpatient Services, In-reach, Outreach, Community, and Emergency.
The Transitional Nurse Practitioner is appointed for a provisional period of up to 3 years to work towards endorsement as a Nurse Practitioner.
