Program Referral Specialist
We are seeking an organized and customer-focused individual to assume responsibility for the centralized intake of all program referrals.
The successful candidate will work closely with Community Managers and Care Coordinators to ensure effective working relationships with internal and external clients, prioritizing tasks and organizing paperwork efficiently.
* Demonstrated organizational skills and experience working in a demanding customer service environment.
* Well-developed communication skills, including effective verbal, electronic, and written skills.
* Advanced computer literacy with experience in client/data management systems.
* Able to prioritize and organize tasks with competing demands.
* Team player with the ability to work independently in a busy environment.

Key Responsibilities:
* Manage program referrals from initial contact through coordination and completion.
* Collaborate with Community Managers and Care Coordinators to ensure seamless communication and efficient workflow.
* Maintain accurate and up-to-date records of client interactions and program activities.
* Contribute to the development and implementation of processes to improve client satisfaction and overall program effectiveness.
