Your Opportunity Reconnect is a community program providing support to at-risk youths aged between 12-18 in the Peel Region. We aim to prevent homelessness through voluntary/self-referrals, emphasise family reconciliation and community engagement. We provide assistance with accommodation, counselling, mediation, referrals, groupwork, practical support, and providing person-centred support. Join us in a vital role supporting youth and families through integrated case management. Provide impactful informal counselling, advocacy, and personalised support to prevent youth homelessness. The hours for this position are either Tuesday to Thursday 8:30am to 4.30pm OR Tuesday to Friday with shorter hours. Never have Monday-itis again! Your Key Responsibilities Respond to referrals of individuals and or families from internal and external services including young people, schools, families, other services and conduct initial assessments of suitability for support. Undertake formal face-to-face assessments of clients, including all necessary paperwork using the approved assessment tools. Induct clients into the service including the program requirements, their rights, and responsibilities. Demonstrate knowledge of the National Principles for Child Safe Organisations. Provide intensive ongoing case management sessions (formal and informal) with clients and review progression against individual support plans and provide informal counselling. Link clients to relevant support services, which will assist in addressing issues including, health, education, employment, recreation, and social networks. Assist clients to source and secure long-term stable accommodation if required. Requirements for Success With a significant background in the child, youth & family and/or social services, you will be confident in your case management and assessment processes.? Other requirements include: A Degree or Diploma in Social Welfare/Social Science or a minimum of three years' experience in a similar role A comprehensive understanding of issues impacting vulnerable and disadvantaged young people in the Peel Region. Experience working with parents and families of disengaged young people. Sound knowledge of and relationship with referral sources, government, and non-government agencies Full driver licence with no restrictions Before starting work with us, you will need to undertake a national police history check, qualifications, and referee checks, and have a clearance to work with children/vulnerable people.
