Clinical Care Specialist Role Overview
We are seeking an experienced Registered Nurse or Allied Health Professional to join our Post Acute Care team as a clinical care coordinator.
The successful applicant will provide comprehensive clinical assessment, needs identification and service coordination to patients following hospitalisation. They will work closely with referrers and community service providers to ensure patients receive appropriate supports.
* Provide virtual care to clients through phone calls, video calls and emails;
* Work closely with referrers and community service providers to coordinate patient care;
* Collaborate with healthcare professionals to develop individualised care plans;
Key Responsibilities:
In this role, you will be responsible for:
* Coordinating patient care and services to ensure seamless transitions between care settings;
* Developing and implementing individualised care plans that meet the unique needs of each patient;
* Fostering strong relationships with referrers, community service providers and other stakeholders to ensure effective communication and collaboration.
Requirements:
To be successful in this role, you will need:
* Experience in case management, care coordination or a related field;
* Strong communication and interpersonal skills;
* Ability to work independently and as part of a team;
* Proficiency in using relevant technology systems and tools.
