Overview
We are seeking a hybrid Social Worker/Discharge Planner to work with our team at Wollongong Private Hospital. You will provide clinical leadership to nursing, medical and allied health providers and deliver an effective discharge planning service, coordinating strategic and operational planning for discharge activities and related services across the hospital. You will deliver personal counselling to individuals and carers as they adjust to their medico-psycho-social situation.
Responsibilities
- Provide social work services including counselling and community follow ups
- Coordinate the patient discharge planning process
- Ensure patients have a smooth transition from hospital to the community with appropriate care and services
- Plan complex discharges with the multidisciplinary team
Skills and Experience
To succeed in this role, you will bring a strong clinical skillset with experience in an acute care hospital setting. You should have effective interpersonal and communication skills (written and verbal), the ability to work in a multidisciplinary, fast-paced environment, a client-focused attitude and a commitment to Ramsay’s core values.
- Degree in Social Work or equivalent
- Eligibility for membership of the Australian Association of Social Workers or equivalent Professional Association
- Minimum of 2 years relevant clinical experience in an acute health care setting
- Extensive knowledge of community services and other resources relevant to this role
- Knowledge of and ability to implement contemporary discharge planning practices
- Demonstrated ability to effectively and efficiently manage a caseload
- Excellent interpersonal and communication skills
- High standard of customer service and a team approach
- Commitment to professional development and maintenance of current evidence-based practice
- Flexible and positive approach to work
- Experience with My Aged Care, the residential aged care process, and the NDIS will be highly regarded

Requirements
- Must provide a National Police Check conducted within the previous 12 months
