Graduate Social Worker
$ $42.60 per hour plus salary packaging
Full time, 80 hours per fortnight with monthly ADO
Fixed term - 2 year contract

Are you a passionate and driven Graduate Social Worker looking for an exciting new challenge? Grampians Health Ballarat is on the lookout for a dynamic Graduate Social Worker to join our innovative team. This full-time, 2 year role offers the chance to work across a variety of programs, including acute, outpatients, inpatient rehabilitation, complex care, community rehabilitation, and more. This position provides an extensive learning and development environment.
The Social Work team provides patients with additional support to improve psychological and social functioning, and we ensure each patients social, emotional and cultural needs are met. We minimise the negative impacts of illness and hospitalisation on patients. The focus is on psychosocial assessments, risk assessments, complex discharge planning, patient and family support.

Key Responsibilities
* Ensure the smooth transition of clients through the health care system, liaising with relevant staff and community agencies to ensure continuity of care for individual patients / clients and their families
* Undertake psychosocial and risk assessments (family violence, elder abuse and child safety)
* Complex discharge planning
* Communicate with a wide range of professionals
* Collaborate in pursuit of patient and team needs/goals
Skills and experience
* Eligibility for practising membership of Australian Association Social Workers Membership (must have a bachelor or qualifying master's degree)
* Ability to monitor, evaluate and modify own performance through reflective practice
* Ability to adapt, be flexible and learn quickly
* Work well under pressure
* Ability to plan and problem solve
* Demonstrated knowledge in Social Work theory and practice skills
* Some knowledge of working in acute/sub acute environments is desirable
