We are seeking a highly skilled and compassionate professional to join our team as an Intake and Assessment Officer.
This role is critical in supporting individuals with health or mobility limitations, ensuring they have access to high-quality support to live independently within their community.
The successful candidate will be responsible for conducting holistic assessments to identify service delivery needs and ensure a positive experience for our clients.
Key responsibilities include:
* Managing enquiries and referrals from clients and stakeholders
* Assessing individual service delivery needs and developing personalized plans
* Maintaining accurate CRM records and communicating effectively with clients and teams
To succeed in this role, you will require excellent communication and interpersonal skills, a calm and professional approach, and a collaborative mindset.
A strong understanding of Tasmania's geography is essential, as well as proficiency in Microsoft Office, web-based systems, and databases.
