What you will do
* Conduct comprehensive nursing assessments and provide tailored care for individuals within the community setting.
* Collaborate with the multidisciplinary team to develop and implement care plans, ensuring high-quality treatment and support.
* Offer guidance and advice to service users and their families, supporting them through their mental health journey.
* Liaise with external agencies and services to coordinate holistic care solutions.
* Participate in team meetings and contribute to the continuous improvement of service delivery.
Requirements of the Community Mental Health Nurse
* A degree or equivalent in Mental Health Nursing is required.
* Significant experience working as a Mental Health Nurse in community settings.
* Current NMC registration is essential to practice as a Mental Health Nurse.
