About the role: Reporting to the Advanced Nurse Unit Manager, GEM at Home & Community Geriatrics, some of your duties will include: Implementing outcome measures appropriate to the client population to evaluate efficacy of therapy programmes in collaboration with the interdisciplinary team Independently assess client’s abilities, problems and needs using a variety of formal and informal assessments as needed Providing care planning, case management and liaison to ensure sustainable social support in place in community. Continually refining and evaluating department protocols, assessment formats, services and processes as part of the auditing/Service Improvement schedule. About you: Ability to critically analyse or evaluate issues related to clinical care in social work in a manner which can be applied to quality activities, service evaluation or research. Experience providing care planning, liaison and case management. Extensive experience in aged care High levels of responsibility, initiative and organisational skills.
