About The Position
Location: Wellington NSW
We are currently seeking a Male Aboriginal Health Practitioner to join our Wellington Clinic Team. The Male AHP directs patient contact and provides a full range of primary care support services under the supervision of the General Practitioners and clinic staff in a way that fully supports a high standard of quality care.
Key Duties
* Liaise with clients, clinic staff, and external health services to support care coordination and referrals
* Assist in delivering culturally appropriate health education and promotion activities
* Monitor and report on Aboriginal health needs and support the implementation of public health programs
* Support GPs and clinic staff with health care planning and primary care tasks
* Collect and manage health data to improve service delivery and outcomes
* Build strong community relationships and promote healthy lifestyles
* Ensure effective communication and continuous improvement across health services
* Engage clients in screenings, tests, and follow-up care as needed
Mandatory compliance requirements of the position
* A Certificate of Aboriginality
* Identify as Male
* Certificate IV in Aboriginal and Torres Strait Islander Primary Health Care (Practice)
* Previous Experience in Aboriginal Health is Highly Preferable
* Understanding and knowledge of specialist medical, allied health and support services locally
* Excellent communication, time management and organizational skills
* Knowledge and understanding of the issues that impact Aboriginal people and Aboriginal communities
* Australian Drivers Licence
* Working With Children Check
* Proof of COVID vaccination
* National Criminal Record Check
