Home Care Support Worker

About the Role
As a Home Care Support Worker, you will provide quality, person-centred care and support services to clients in their own homes. You will assist clients to maintain their independence, dignity and quality of life in accordance with their care plans and the Aged Care Quality Standards.
This role involves travelling throughout the Southern Highlands to deliver care services to clients in the community.
Key Responsibilities
You will provide support in line with individual care and services plans, including:
* Personal care (showering, grooming, dressing)
* Mobility assistance and transfers
* Meal preparation and nutrition support
* Medication reminders (within scope)
* Continence support
* Light housekeeping, laundry and grocery shopping
* Companionship and social support
* Supporting community participation
* Monitoring and reporting changes in client wellbeing
* Maintaining accurate documentation
All services are delivered in alignment with Anthem Care policies, WHS requirements and the Aged Care Quality Standards
Essential:
* Certificate III in Individual Support (Aged Care/Home Care) or equivalent
* Experience in home care or aged care
* Caring, compassionate and respectful approach
* Ability to work independently
* Strong communication skills
* Current National Police Check
* Current driver's licence
* Reliable vehicle (travel required)
Desirable:
* First Aid Certificate
* Food Handling Certificate
* Dementia training or additional aged care courses
