Overview
Social Worker Senior - Women, Children and Family Division at West Moreton Hospital and Health Service.
This role will provide clinical practice, advice, and consultation to multidisciplinary teams to enhance service delivery and support the continuum of care from hospital to the community. All clinical services are provided in line with the Social Work professional code of ethics and practice standards. The role uses high-level knowledge and skills to undertake clinical service delivery where clinical needs are complex, including assessment to identify clinical risk, high-level advocacy, and leadership to the clinical team on the utilisation of relevant legislation and policy to achieve safe patient outcomes.
About You
Our ideal candidate will demonstrate:
- High-level expertise in delivering assessment, interventions, and independent decision-making to provide evidence-based assessments and interventions for families with complex needs, focusing on at‑risk children and families and bereaved parents, to ensure a high standard of patient-centred care and trauma-informed practice.
- High level of qualified leadership applied to a complex caseload of adult patients, children and families within an interprofessional team, with independent practice and professional decision making, accessing clinical practice supervision as required.
- Effective written and oral communication in providing multidisciplinary care in a complex environment, including psychosocial assessments, clinical documentation, consultation, reports, and high-level advocacy to contribute to patient care.
- Clinical leadership in supervising and developing staff, supporting evidence-based practice to improve processes, and ongoing professional development.
- Clinical advice and applying clinical expertise to integrate the social work perspective and collaboration to improve health outcomes.
Mandatory Requirements
- Vaccinated against, and remain vaccinated against, certain vaccine preventable diseases (VPD) as outlined in Health Employment Directive NO. 01/16.
- Appointment requires proof of qualification and registration or membership with the appropriate Australian registration authority or association for a Social Worker; eligibility for AASW membership.
- This role involves providing health, counselling and support services that may involve children and will require a blue card.
