OCCUPATIONAL THERAPIST
Kanda is seeking an experienced Full time Occupational Therapist to join our community nursing team to provide care for, and to improve health outcomes for our Veteran (DVA) and Aged Community living at home.

We provide care, support and home nursing services to clients throughout Australia across a variety of programs including our transitional care programs in Adelaide.
The Occupational Therapist will work closely with our Transitional Care Program Coordinators to grow & support our business throughout South Australia
About The Role:
In this Full Time role your primary responsibility will be assessing your client’s strengths and abilities and tailoring a goal driven program to assist the client in achieving their independence goals. You will manage your own varied clinical caseload to provide high quality and evidence-based solutions.
Working with the Transitional Care Program (TCP) coordinator to ensure clients are meeting their therapy goals and where necessary, reviewing and adapting programs to suit their needs. you will provide reports and regular updates to the multidisciplinary team.
Key Requirements
Current registration with AHPRA
A degree (or higher) qualification in Occupational Therapy
Knowledge, skills and experience with Aged person-centred services
Genuine commitment to helping older individuals retain their independence
High level of communication skills that display a commitment to care and empathy for clients and staff
The ability to work autonomously as well as part of a team
Willing to obtain the relevant screening clearances
Your own reliable vehicle
First Aid Certificate (with CPR updated annually)
Responsibilities:
Supporting clients within their own homes
completing thorough client assessments
Assessing needs for home modifications and equipment to assist client to continue living independantly
Liaising with other healthcare professionals to encourage a holistic approach to treatment
Implementing functional retraining programs including referring to allied health assistants
Generating care plans, reviewing outcomes and goal updates, and contributing to the planning, implementation, promotion and evaluation of services for the individual
