My Home Supports is expanding across the Geelong region, and we’re looking for casual/ permanent part-time clinicians (recent grads welcome) to join our growing allied health team. Our model is community-based, personalised, and rewarding — you'll work 1:1 with older clients in their own homes to help them stay safe, independent, and thriving.

What You'll Do
* Provide individualized care to older adults in a community setting
* Support older Australians to remain active, independent and safe in their own homes
* Complete evidence-based functional and mobility assessments
* Develop client-centred goals and treatment plans
* Prescribe assistive devices (i.e. transfer or mobility aids) to improve safety and independence
* Falls prevention and risk minimisation
* Pain management and musculoskeletal care
* Collaborate with a multidisciplinary team (Physiotherapy, Occupational Therapy, Nursing, Dietetics, Clinical Coordinator)
* Empower clients to achieve functional, mobility and independence goals
* Document clinical findings and reports with accuracy and compassion
* Work closely with care partners from local in-home care providers
Who We're Looking For
* AHPRA-registered Physiotherapist
* Must have current drivers’ license and own reliable car
* Willingness to drive across Geelong region including Lara, Bannockburn and northern suburbs of Geelong
* Experience in community care and working with seniors or willingness to learn
* Confident working independently with the support of a team behind you
* Commitment to best practice, kindness, and high-quality care

My Home Supports is a locally owned and operated Allied Health & Nursing service, providing high-quality care in client’s home for individuals covered by Support at Home (SAH). We are a close-knit local multidisciplinary team including Physiotherapists, Occupational Therapists, Dieticians and Registered Nurses. We believe in empowering people to live well at home — and we support our team to live well, too.
