The Transitional Nurse Practitioner (TNP)/ Nurse Practitioner (NP) will provide a high level of evidence-based nursing care within the specialty area of Palliative Care - End of Life nursing.

The TNP/NP role will improve access of care and address gaps in existing health care services through the provision of a flexible and innovative model of care.

The TNP/NP will provide clinical and professional leadership as an expert in the clinical field and will undertake comprehensive health assessments and collaborate and consult with other members of the health care team to plan, implement, and evaluate care. This integrated approach will support best practice and maximise patient care outcomes.

A Nurse Practitioner must have current registration as a Registered Nurse with the Australian Health Practitioner Regulation Agency (AHPRA) who is endorsed to practice as a Nurse Practitioner by the Nursing and Midwifery Board of Australia.

A Transitional Nurse Practitioner must be a Registered Nurse (AHPRA) working towards Nurse Practitioner endorsement with at least 5 years post graduate experience and at least 3 years in the relevant field. The TNP is appointed for a provisional period of up to three (3) years until the applicant meets the NMBA requirements for Nurse Practitioner endorsement.
