About the Young People's Transition Program. The purpose of this initiative is to improve community safety and reduce reoffending by supporting young people who are transitioning from detention back into their communities.
This program aims to bridge the gap between detention and the community, providing young people with the skills and resources they need to turn their lives around and avoid further offending.
We are seeking passionate and dedicated practitioners to join our team in delivering this program. As a practitioner, you will play a critical role in supporting young people aged 10-17 who are involved in the youth justice system.
The key responsibilities of this role include:
* Delivering best practice client-focused support through therapeutic interventions, intakes assessments, case planning and goal setting;
* Providing trauma-informed and culturally responsive case management tailored to the needs of young people and their families;
* Supporting young people and families to understand and meet expectations of court orders, case plans and service response plans;
* Delivering structured mentoring to address criminogenic needs by building life skills, relationships and community connection;
* Conducting safety planning and risk assessments and developing appropriate risk mitigation strategies;
* Maintaining accurate and timely case notes, reports and documentation in line with legislative contractual and organisational requirements;
Required skills and qualifications:
* Degree in Social Work, Human Services, Behavioral Science, Psychology or similar;
* Current Blue Card Working with Children Check;
* Current National Police Check;
* Queensland Driver License;
* Willingness and ability to travel within the service region;
* Willingness and ability to work flexible hours, including after-hours and on-call support;
