About the role:
The role combines the integrated team care and shared medical appointment coordination that
will undertake one function to support the member services in community to successfully
mobilise effective health outcomes.
Integrated team care
The role is responsible for supporting the integrated team care (ITC) workforce, including care
coordinators and outreach workers, employed in AMSANT member services who receive
ITC funding. The ITC program aims to ensure a focus on Aboriginal health and chronic disease,
contribute to closing the gap in life expectancy, and improve care integration and coordination.
The role works in close collaboration with the PHN, AMSANT, organisations that employ ITC care
coordinators and outreach workers, and the care coordinators and outreach workers themselves
to actively support opportunities that enhance the improvement of Indigenous health through care
coordination.
Shared medical appointments
You will be responsible for coordinating the shared medical appointments/medical yarn ups
(SMA/MYU) program, working with AMSANT member services and key stakeholders in chronic
disease management. This position will focus on supporting Aboriginal community controlled
health services to be innovative in the delivery of comprehensive chronic disease management
and treatment through coordinated care for patients in their community.
Responsibilities
* Project manage and coordinate the SMA/ITC program in consultation with relevant providers and key stakeholders.
* Coordinate meetings and forums with our member services and relevant stakeholders, including the annual integrated team care (ITC) forum in partnership with the NTPHN and internal programs.
* Work in collaboration with the SMA/ITC program staff to provide support, training opportunities and resources to our member services and their workforce.
* Provide input into SMA/ITC initiatives that are relevant and appropriate to AMSANT and its members.
* Participate in all relevant AMSANT meetings and forums as required.
* Represent AMSANT/member services at key external meetings and forums as requested by WALS manager.
* Provide succinct and timely reports as required for the SMA/ITC programs.
Selection criteria (qualifications and attributes)
Essential
* Sound knowledge and understanding of chronic conditions in Aboriginal health, including a sound understanding of the social determinants of health and how these affect chronic conditions management.
* Demonstrate knowledge and understanding of Aboriginal and Torres Strait Islander peoples, cultures, histories, politics and principles of the Aboriginal community controlled health sector.
* Tertiary qualification and/or experience in health or human services or the equivalent level of expertise in the Aboriginal community controlled health sector. This includes across the health sector, or member-based organisations gained from a combination of experience, training, skills and personal attributes.
* Proven ability to coordinate projects and simultaneous activities, including the ability to work independently to coordinate and deliver multiple projects with multi-disciplinary teams.
* Ability to effectively engage with a diverse range of organisations and individuals at all levels, informal and linking trainees to professional support.
* Ability to regularly travel within the Northern Territory to regional and remote communities by road or light aircraft.
Desirable
* Previous experience working in Aboriginal primary health care in the Northern Territory.
* Knowledge and experience in health policy, specifically in chronic conditions.
* Knowledge of and understanding of the work of AMSANT.
