About the role
As a Full Time Support Coordinator with Quantum Navigators P/L, you will play a vital role in assisting people with disabilities and their families to access and coordinate the services they need to live independently and achieve their goals. Based in the beautiful Coolum Beach, QLD, this role offers a flexible work arrangement to help you achieve the right work-life balance.
What you'll be doing
1. Assess clients' support needs and collaborate with them to develop individualised support plans
2. Coordinate the delivery of a range of community, health and other support services to ensure clients' needs are met
3. Monitor and review clients' support plans regularly to ensure they continue to meet their goals
4. Advocate on behalf of clients to ensure they access the services and funding they are entitled to
5. Maintain detailed client records and documentation in line with organisational and regulatory requirements
6. Liaise with a wide range of service providers, government agencies and other stakeholders to facilitate client support
What we're looking for
1. Relevant tertiary qualification in disability, aged care, social work or a related field
2. Minimum 2 years' experience in a similar support coordination or case management role
3. Strong understanding of the disability services sector and the National Disability Insurance Scheme (NDIS)
4. Excellent communication and interpersonal skills, with the ability to build rapport with clients and stakeholders
5. Well-developed problem-solving and decision-making skills
6. Proficient in using client management software and MS Office applications
7. Current driver's licence and access to a reliable vehicle
