* Join HammondCare's Specialist Dementia Care Program (SDCP) – a Commonwealth initiative
* Supporting people with Tier 6 BPSD (Behavioural and Psychological Symptoms of Dementia) through a transitional care model. This role is pivotal in delivering clinical oversight, behaviour support, and transition planning, working closely with:
* Clinical In‑Reach Team (Psychogeriatrician & CNC)
* Skilled internal multidisciplinary team and interdisciplinary team
* Other 7 SDCP managers across three states
* Dementia Support Australia
* Residential Manager and Manager Dementia Expertise for strong operational and program support
* You will also oversee the Step‑Down Unit, collaborate with Dementia Consultants, and lead strategies to understand and address the root causes of behaviours.
What You Will Do
* Provide clinical leadership and behaviour support expertise.
* Mentor and coach teams in complex dementia care.
* Drive proactive behaviour support strategies.
* Partner with health specialists and external stakeholders.
* Ensure person‑centred, strengths‑based, relationship‑focused care.
What We Are Looking For
* Registered Nurse with specialist dementia or mental health expertise.
* Strong leadership, influencing and coaching skills.
* Experience managing complex behaviours and psychological symptoms of dementia.
* Excellent analytical, negotiation and communication skills.
