Population and Community Health (PaCH) is part of South Eastern Sydney Local Health District.
PaCH provides community health services that give care to individuals, families and carers, as well as population health services that focus on the whole community or key groups within the community.
PaCH's services are provided at the District's hospitals, community-based facilities including child and family health centres, community health centres and specialist clinics. We work with other service providers across South Eastern Sydney Local Health District, including hospitals and mental health services.

The Role
The vision for South Eastern Sydney Local Health District (SESLHD) is 'exceptional care, healthier lives'. SESLHD is committed to enabling our community to be healthy and well, and to providing the best possible compassionate care when people need it.
SESLHD's Sexual Health and Blood Borne Virus Services (SHBBV) provides leadership, innovation and research in the sector. Services within SHBBV are involved in the prevention, diagnosis, and treatment of sexually transmissible infections (STIs) and blood borne viruses, as well as the health and wellbeing of those at risk of such infections. Across these services, which include both SESLHD and statewide teams, care is provided via fixed locations and outreach networks to a client base of approximately 28,000 individuals. SHBBV has highly skilled and diverse staff including nursing, medical, allied health, health promotion, administrative and technical staff. It is the largest SHBBV service in Australia.
The Kirketon Road Centre (KRC) is a targeted primary health care service located in Kings Cross, Sydney, involved in the prevention, treatment and care of HIV/AIDS and other transmissible infections among people who inject drugs, sex workers, people experiencing homelessness, 'at risk' young people and Aboriginal people.
This position is responsible for the management of the Counselling Unit and the delivery of high quality counselling services for KRC clients. These include the provision of crisis and support counselling, opioid treatment program case management, social welfare assistance and health promotion services at KRC services and in outreach settings. KRC has an eclectic approach underpinned by the harm reduction framework, providing services in a non-judgemental way. KRC encourages self-determination, while ensuring that clients' lifestyle choices are well informed.

Tertiary qualification in social work, psychology or equivalent in accordance with Schedule C of the NSW Health Service Health Professionals (State) Award.
Minimum of 5 years' post-graduate clinical experience with demonstrated skills in the provision of counselling, case management and/or psycho-social support to clients with complex needs /co-morbidities, including the ability to proactively engage with KRC target populations.
Demonstrated leadership and experience in the management of staff including recruitment, rostering, performance management and conflict resolution.
A commitment to and an understanding of the harm reduction, primary health care and health promotion approaches
Demonstrated ability to work flexibly in unpredictable and unstructured environments
An understanding of child protection policy frameworks
Experience in quality improvement and other service evaluation initiatives
Current NSW Driver's Licence
