About the role:
Our Toowoomba Home and Community service is seeking a Registered Nurse to provide community nursing across the Toowoomba region. You will travel to clients' homes and provide them with comprehensive nursing assessments and case management and apply clinical knowledge to implement planned nursing care.
We can accommodate both permanent full-time and permanent part-time positions, expected hours of work will be across Monday-Sunday. You will be required to travel across the Toowoomba region. You will get a kilometre allowance for travel and on call payment in addition to the hourly rate ($42.79-$52.29 dependent on experience).
Position duties:
* Conduct and document assessments accurately, representing each client's current status and requirements, while complying with Anglicare policies and procedures.
* Develop holistic and effective care plans from assessments.
* Participate in the completion of documentation as required under the ACQSC Quality Standards and the Aged Care Act.
* Effectively communicate with clients and other key stakeholders in ongoing development and review of care delivery.
* Regularly review each client's progress against their current care plan and modify as indicated by changed care needs.
* Liaise with Anglicare colleagues and other health care service providers involved in client care plans.
* Effectively communicate with clients and their families/carers and keep them informed of relevant changes to a client's progress and care plans.
* Deliver care in a safe and competent manner.
* Ensure best practice, evidence-based resources and knowledge are utilised to deliver care.
* Document the care provided as per organisational documentation guidelines and client management systems.
* Coordinate, supervise and monitor delivery of care by care staff (including but not limited to ENs, HCWs and other support staff) and delegate tasks appropriately.
* Administer all medications in accordance with the Drug Health and Poisons Act and the relevant medical practitioner's prescribed orders.
* Participate in case conferences with clients, their families and relevant service providers.
Position requirements:
* Current National Police Certificate
* Current Drivers Licence and own reliable vehicle
* Current CPR Certification
* Degree in Nursing or equivalent qualifications and experience to Registered Nurse level
* Current AHPRA registration and evidence of recent practice
* Experience working as a registered nurse in community aged care.
* Demonstrated ability to coordinate and deliver quality nursing care, whilst working within a risk management framework and in accordance with professional standards and conduct (e.g. infection control, Codes, nursing competencies, Standards and legislation) as relevant to community nursing practice.
* Proven ability to work independently with effective time management and problem-solving skills.
* Demonstrated ability to build and maintain strong relationships, open communication and a positive environment within a multidisciplinary team.
* Demonstrated active involvement in quality activities for service improvement in a clinical setting.
* Previous aged care experience, highly desirable.
