About the role

Are you a junior doctor looking for a unique and flexible role that offers unparalleled learning opportunities and skill acquisition fit for a future career in General Practice or Geriatric Medicine? Gem at Home seeks a dynamic junior doctor who is eager to make a difference in the lives of patients. You will work closely with our Advanced Trainee and Geriatrician who will provide mentorship and support as you navigate community-based healthcare.

You will have the flexibility to choose your working days (if part-time). You will be based at our office/hub located at Sunshine Hospital, joining a supportive and collaborative team dedicated to delivering high-quality care.

Gem at Home

* Aim to enable earlier discharge of patients over 65 whose family or carers support them at home while they continue functional and cognitive improvements after hospital admission
* Provide comprehensive geriatric care and assessments at the patient's home. Referred by Western Health acute and subacute wards via SNAP (Subacute and Non-acute Assessment and Pathways) and external networks
* Up to 20 virtual beds with length of stay varying from 1–4 weeks depending on patient progress
* Full Time – Monday to Friday (no weekends)
* Part Time – Monday to Friday (working days negotiable)

Key responsibilities

* Providing patient care as part of a multidisciplinary team
* Conducting phone calls and telehealth with patients and other specialties/GPs
* Liaising with inpatient teams
* Completing admissions and clinical reviews in the patient's home and producing discharge summaries in a timely manner
* Possess Australian Driver's license and be willing to perform home visits with team support

About you

* Current APHRA Registration as a Medical Practitioner
* Post-graduate Year 3 or above at the time of commencement
* Commitment to ongoing professional development and quality improvement
