This is a critical role within the organization, requiring an individual with exceptional skills and experience in disability support. The successful candidate will provide expert care coordination, case management, and navigation for individuals with disabilities, ensuring seamless transitions between healthcare settings.
The Disability Navigator will work closely with multidisciplinary teams to develop and implement comprehensive care plans, leveraging their advanced clinical reasoning and negotiation skills to drive positive outcomes.
Key responsibilities include:
* Providing expert advice on disability-related matters
* Coordinating and communicating complex care plans within the healthcare context
* Building effective relationships with stakeholders, including patients, families, and healthcare professionals
* Utilizing strong advocacy and negotiation skills to secure necessary resources and services
* Maintaining accurate records and reporting requirements
To be successful in this role, candidates must possess:
* Tertiary qualification in Social Work or a relevant Allied Health profession
* At least 3 years' experience as an Allied Health Clinician in disability support
* Advanced clinical reasoning and negotiation skills
* Strong understanding of navigating the NDIS and alternative disability-related supports
* Ability to prioritize caseload, manage stress, and work autonomously
