Support Facilitator Role
As a Support Facilitator, you play a vital role in providing short-term non-clinical support to individuals at risk of suicide. The Way Back service aims to support people after a suicide attempt or during a crisis.
* Promote a supportive environment based on trust, compassion, empowerment, and respect while encouraging personal growth and conflict resolution.
* Provide recovery-oriented support including capacity building, community connections, and collaborative development of Wellbeing Plans and safety strategies.
Key responsibilities include:
* Conducting risk assessments using the STARS tool, participating in case reviews, and escalating client or organisational risks appropriately.
* Working with internal teams, hospitals, and external services for continuity of care and support clients' transitions and connections to health and community services.
* Maintaining accurate records using digital systems and ensuring clear communication within the team and with stakeholders.
* Engaging in supervision, performance reviews, and continuous improvement efforts, contributing ideas and staying informed of policies and best practices.
