Clinical Coordinator
We are seeking a highly skilled and motivated Registered Nurse to join our team as a Clinical Coordinator. This is an exciting opportunity to work in a dynamic and supportive environment, coordinating the delivery of service to clients with complex care needs.
As a Clinical Coordinator, you will play a key role in developing and implementing client care plans, providing clinical guidance to our clients and network of support staff, and managing daily branch operations. You will be responsible for ensuring optimal branch performance, increasing branch net revenue, and taking part in networking and industry related events to improve outreach.
Key Responsibilities:
1. Day-to-day running of the branch
2. Ensuring optimal branch performance and increasing branch net revenue
3. Taking part in networking and industry related events to improve outreach
4. Developing and reviewing client care plans and clinical documentation
5. Identifying, assessing, and remediating any clinical matters, obstacles and barriers impacting on clients' care
6. Facilitating inductions, education and training workshops for our support staff to upskill and enhance professional development and conducting clinical competency assessments for our support staff
7. Visiting clients regularly to maintain relationships built on trust and genuine connection
8. Managing stock orders for our clients and reviewing client progress notes
9. Undertake Work Health & Safety compliance
10. Managing clinical compliance for all clients to meet Attendant Care Industry and National Disability standards
11. Liaising with allied health, support coordinators and other external stakeholders
Requirements:
1. Current AHPRA registration as a Registered Nurse (with nil restrictions)
2. Passion for health education, demonstrating skills in health assessment, planning and goal setting
3. Strong communication skills (both oral and written), organisational and computer skills
4. Ability to work remotely and autonomously when required
5. Understanding of the NDIS and other funding bodies (desirable)
6. Results orientated - proven success record of achieving targets/KPIs
7. Knowledge of and commitment to total quality management
