About The Opportunity

The Aboriginal Chronic Care Team (ACCP) is a small but mighty multidisciplinary team made up of Aboriginal Health Workers/Practitioners, Care coordinators, Dietitians, Admin, CNCs, Clinical Team Leader, Facility Manager and ACCP Program Manager.

The team covers the whole SWSLHD with 3 office/clinic sites across the district – Wellama (Bankstown), Budyari (Miller) and ACCP offices (Miller).

The team has a big focus on not only the wellbeing of our clients but also each other. The team work collaboratively in the provision of specialist services to Aboriginal patients and their families with chronic and/or complex needs in a culturally safe, patient centered way to facilitate optimal health outcomes.

As well as having access to our onsite clinics, we provide Integrated Team care, to assist Aboriginal and Torres Strait Islander people in South Western Sydney who are living with a chronic condition to navigate the Health system in partnership with their GP.

We do this through providing holistic, culturally appropriate health care to:

- Improve health outcomes/lifestyles.
- Increase early detection and intervention.
- Increase knowledge and awareness through education of risk factors associated with chronic disease.
- Work in partnership with Aboriginal Health Services, GPs and other healthcare providers.
- Close the Gap in life expectancy.

With the Ultimate Goal of Client Self-Management we aim to 'Close the Gap' in health disparities for Aboriginal people by addressing chronic and complex health conditions in a culturally safe, patient-centered way.

Our service area spans the Dharawal, Gundungurra, and Darug Nations, covering Bankstown, Fairfield, Liverpool, Campbelltown, Camden, and surrounding Macarthur suburbs, as well as the Wollondilly and Wingecarribee areas.

What You'll Be Doing

The position is responsible for the provision of culturally safe, patient-centered care coordination services to Aboriginal patients and their families with chronic and/or complex needs. The position will work collaboratively with an Aboriginal Health Worker to provide assessment, care coordination, and education to patients, families and carers that take into account clinical, psychosocial and cultural needs.

SWSLHD Primary & Community Health provides a comprehensive range of community-based services designed to improve and maintain the health and wellbeing of individuals and communities across South Western Sydney. These services include prevention, early intervention, assessment, acute and post-acute treatment, health maintenance, and continuing care. The team supports diverse populations, offering specialised programs for children, young people, older adults, and at-risk communities. With over 800 dedicated staff, including Aboriginal Health Workers, nurses, doctors, and allied health professionals, the service ensures high-quality care close to where people live.

- Aboriginality – this position has been advertised under the SWSLHD Aboriginal Targeted Recruitment Program. This means Aboriginal and Torres Strait Islander people, where found suitable, will be given higher priority. Do you identify as Aboriginal and/or Torres Strait Islander? If so, please tell us about your Aboriginal and/or Torres Strait Islander heritage, including a brief description of your family and community connections.
- Current registration with AHPRA as a practicing Registered Nurse, Psychologist, Physiotherapist or Occupational Therapist or professional qualifications in Social Work, Exercise Physiology or Dietetics which provides eligibility to the relevant professional association. CNS Grade 2: Relevant post-registration qualifications and at least 3 years’ experience working in the clinical area of the employees specified post-graduate qualification, or such other qualifications or equivalent experience deemed appropriate by the public hospital or public health organisation.
- How has your experience delivering culturally safe chronic and complex care to Aboriginal communities shaped your understanding of the factors influencing their health, and how do you apply health coaching and goal-setting techniques to support patients with complex needs?
- Can you describe how your communication skills, critical thinking, and cooperative approach with patients, carers, and multidisciplinary teams contribute to responsive care delivery, while also supporting risk management, quality improvement, and workplace safety?
- Applicants must possess an Australian C-Class driver's license (including P1 or P2). Those who hold a C class license (P1 or P2 included) from a state other than NSW must obtain a NSW C class (P1 or P2 included) license within three months of appointment. Learner's licenses are not permitted.
