Kalwun Development Corporation Ltd is an Aboriginal & Torres Strait Islander community controlled organisation currently seeking to fill the following vacancy within our Seniors Services division. Kalwun Seniors Services is a provider of the Commonwealth Home Support Programme and Home Care Packages, and runs a day respite Wellness Centre.
About the Role
The Team Leader (Home & Community Support) will be responsible for mentoring; training and supervising a team of Community Support Workers. A base-level of leadership duties forms part of this role with the support of the Manager (Service Delivery). The position also supports direct service delivery by attending to ad-hoc client shifts where operational need requires. The position holder will work collaboratively with the Manager (Service Delivery) in providing Kalwun clients with quality care, actively trying to maintain the skill level, independence and mobility of individual clients while providing respite support to their family/carers.
Position Duties
Lead a team of Community Support Workers to deliver exceptional services to our clients
Assist the Manager (Service Delivery) to oversee the daily operational workflow activities with a view to driving efficiency within the team and process improvements across the business, to achieve best practice.
Lead by example as a hands-on team leader in the day-to-day supervision and mentoring of the team ensuring a highly skilled, informed, confident and competent team
Proactively attend support worker shifts to facilitate on-the-job training and supervision
Assist with annual customer satisfaction surveys
Participate in Quality Audit Reviews from external agencies and maintain service standards that will achieve all standards met
Participate in the implementation of new services in line with the operational plans and strategic direction
Follow standardised documentation, policy and processes including updating TRACCS
Liaise with community care workers regarding implementation of changes to a client care plans where direct care/services are modified as part of their agreement
Work collaboratively with clients, their carers, family, and/or advocate along with other Kalwun staff, health professionals and other external stakeholders to maximise Client’s independence and their ability to remain living in the community.
Essential Selection Criteria
Detailed understanding of Home Care Package, CHSP, and Support at Home program frameworks
A working knowledge of Microsoft Office products including Word, Excel and Outlook and demonstrated ability in the use of client/data management systems
Knowledge of and experience working in the Aged Care and Community Sector
Knowledge of and experience working within the Aged Care Quality Standards.
