In recent years, Cootharinga has merged with CPL and so you will work across teams representing Cootharinga and CPL.
We're seeking an experienced Service Facilitator to join our passionate team and help us deliver exceptional outcomes to our clients. Your role will be to facilitate and administer the provision of high‐quality support services to clients with various disabilities, in line with the individual's service plan and our organisational strategy.
What would I be doing each day?
* Ensuring the provision of high‐quality services and support to clients within NDIS, regulatory & organisational guidelines.
* Liaising with each client's external stakeholders to ensure appropriate access to general and specialist health care, allied health support and ensuring all health care plans are implemented effectively.
* Proactive and supportive client and family engagement to support client decision making and control, and ensuring excellent communication with each client's informal networks.
* A focus on continued improvement to enhance the service experience for each client and family.
* Ensuring the health and wellbeing of your direct support worker team and contributing to a diverse and inclusive workplace.
* Daily management of rosters to ensure accuracy and to provide for timely claims for payment under the NDIS system.
* Assisting clients in managing their finances in accordance with policies and procedures and where applicable Public Trustee guidelines.
* Supervising, coaching and mentoring of your Direct Support Worker team.
* Facilitating the effective communication and coordination between services, staff and stakeholders providing support to clients.
* Being willing to be part of a rotating after‐hours backup and On Call roster, paid as per SCHADS award.
What would I need to be considered?
* Experience managing disability support services, complex supports needs and understanding of NDIS Plans.
* Commitment and drive for the highest level of customer service.
* Pro‐active, energetic approach to a dynamic ever‐changing environment!
* Experience and skill in team supervision, leading people to achieve.
* Highly effective interpersonal and communication skills.
* Able to work autonomously and collaboratively within teams.
* Well‐developed administrative and financial management skills.
* Current 'C' class driver's license and reliable transport.
* Current First Aid and CPR.
* Competency with Microsoft Office and computer systems.
* Blue card and NDIS worker screening clearance.
