We are seeking experienced and motivated Registered and Enrolled Nurse to join our dedicated Community Nursing team based from Tooleybuc. The following positions are available:
Enrolled Nurse: Temporary Part-time, up to 12 hours per week.
As the Enrolled Nurse, you will:
* Deliver safe, person-centred, evidence-based nursing care
* Assess, plan and evaluate care in collaboration with the Registered Nurse
* Provide clinical care, education and health promotion
* Build strong therapeutic relationships and work collaboratively within the multidisciplinary team
* Recognise and respond to deterioration, escalating care as required
* Support safe transitions of care and continuous improvement activities
* Work within scope of practice, including delegated activities, and support AINs, students and learners

Registered Nurse: Temporary Part-time, up to 20 hours per week.
As the Registered Nurse, you will:
* Provide comprehensive, coordinated and person-centred nursing care to individuals and families within their homes and community settings. Provide a client goal focused service to empower them to achieve their health goals
* Delivering a broad range of services including chronic disease management, wound and catheter care, palliative care, health prevention, health promotion and carer support. Post acute services such as CVAD management, VAC management, Chemotherapy disconnections, drain management
* You will work collaboratively with consumers, carers, General Practitioners and multidisciplinary teams to ensure continuity of care and optimal health outcomes.
