About the Role
We are seeking a dedicated Support Coordinator / Community Access Team Leader to join our team in Mirrabooka.
In this dual role, you will:
Support Coordination – Work alongside NDIS participants to help them understand their plans, connect with the right supports, and achieve their personal goals.
Team Leadership – Provide guidance, supervision, and mentoring to our community access staff, ensuring high‑quality service delivery and operational efficiency.
This is a diverse, hands‑on role where no two days are the same. You'll combine frontline client support with team leadership and compliance responsibilities, making a tangible difference in people's lives.
Key Responsibilities
Engage with participants to identify goals, build recovery‑focused plans, and connect them with the right supports.
Coordinate services across health, education, housing, and community providers to maximise outcomes.
Provide crisis response, risk management, and problem‑solving to ensure participant safety and wellbeing.
Lead, mentor, and roster a team of support staff, ensuring services align with NDIS plans.
Maintain accurate documentation, budgets, and reports in compliance with NDIS standards.
Promote community engagement and networking opportunities to strengthen participant inclusion.
What We're Looking For
Relevant qualifications in Disability, Social Work, Allied Health, or Community Services.
Experience in support coordination, case management, or community services leadership.
Strong knowledge of the NDIS and related compliance requirements.
Excellent communication, problem‑solving, and organisational skills.
A professional, positive, and adaptable approach.
Proficiency in Microsoft Office and client management systems.
