As a Care Manager, you will be responsible for overseeing and coordinating all aspects of client care, ensuring a seamless and positive experience for both potential and existing clients. This position provides leadership and guidance in managing client interactions, working closely with the office team to meet the needs and best interests of the client while achieving key operational goals.

Key Responsibilities
* Planning, managing, coordinating, and reviewing client goals.
* Actively communicate with team members and clients to develop care plans.
* Formalise agreements with the client and client’s family about the care services to be provided and costs involved.
* Accurately document all interactions held with client, family or other stakeholders about the client and share relevant information with appropriate team members including feedback about caregiver performance.
* Meeting with clients and their families face to face to discuss and review current care plans and goals.
About you
* Qualifications in Community Services preferred
* Aged Care and Case Management experience a must
* Experience with Home Care Packages, Support at Home or Aged Care Case Management
* Great customer service manner
* Strong administration skills in Microsoft Office & Database experience
* Highly efficient and organized with proven time management skills
* Have a full Australian Driver's Licence
