The Opportunity
You will work alongside clients, carers, and a multidisciplinary team to deliver person-centred care that supports independence, health, and quality of life. Your role will involve coordinating and reviewing tailored care plans across aged care and disability programs, ensuring every interaction counts.
* Complete holistic assessments and co-design care plans aligned to client goals and clinical recommendations.
* Coordinate and monitor services under CHSP, Support at Home, QCSS, and NDIS.
* Help clients and carers navigate systems, provide informed consent, and share feedback.
* Collaborate with internal/external clinicians for complex needs and referrals.
* Keep accurate records and report outcomes to funding/quality frameworks.
* Build strong, respectful relationships with clients, families, and community stakeholders.
* Travel to outreach locations as required.
What You'll Bring
* Experience in case management or coordination in aged care and/or disability services.
* A client-centred, strengths-based approach and cultural humility, particularly when working with Aboriginal and Torres Strait Islander peoples.
* Confidence with mobile tech and digital health records; MS Office proficiency.
* Strong communication, documentation, and time-management skills.
* Ability to work autonomously and within a multidisciplinary team.
Mandatory Requirements
* Current First Aid & CPR certification.
* National Police Check and NDIS Worker Screening.
