* The ideal candidate will champion integration between health and housing sectors to prevent homelessness, make it rare, brief, and non-recurring.
In this pivotal role, you'll play a key part in building a community-wide response that strengthens pathways out of homelessness.
Key Responsibilities
* Coordinate multi-agency service coordination meetings and community engagement workshops aligned with Zero methodology
* Track actions and outcomes for individuals on the By-Name List using data tracking tools
* Collaborate with healthcare services to align housing and health pathways
* Strengthen partnerships with local stakeholders to advance systemic change
As a Housing Coordinator, you will have demonstrated experience in housing, homelessness, or community services, strong facilitation, coordination, and stakeholder engagement skills, proficiency in data tracking tools, and understanding of Housing First principles and Zero methodology.
