About the role:
As a Care Navigator with Ballarat Community Health's Seniors Lifestyle Program, you will play a key role in supporting older adults to live independently, safely and confidently in their own homes. Working within a warm and collaborative multidisciplinary team, you will provide person-centred care coordination, guiding participants through tailored plans that build capacity, enhance wellbeing and strengthen social connection.
In this rewarding role, you will help individuals reduce or slow the impacts of frailty, minimise avoidable hospitalisations and achieve meaningful, self-defined goals. Through proactive engagement, coaching and partnership with allied health, nursing, general practitioners and community providers, you will ensure each person receives timely, holistic support aligned with their needs and aspirations. Your work will make a real difference - empowering older people to maintain independence, stay active and improve their quality of life.
What you will bring to the team:
Strong case management capability, including experience in care planning, community aged care, and supporting older adults with complex or changing needs.
Exceptional interpersonal and communication skills, with the ability to build trust, show empathy and work collaboratively with diverse clients, families and service providers.
Clear, concise and professional written communication, including report writing and documentation aligned with clinical and program requirements.
Confident use of digital tools and systems, including Microsoft Office, clinical management software, PRODA and My Aged Care.
Highly organised, proactive and solution-focused, able to balance competing priorities, work independently and maintain excellent attention to detail.
