About the role
Female‐only role to provide support services in the Lismore region.
Provide hands‐on support to people living with disability in a home setting, helping with personal care, meal preparation, medication and daily tasks while encouraging independence. This permanent part‐time role offers 48 hours per fortnight across a 24/7 roster.
Responsibilities
* Collaborate with customers to develop individualised, person‐centred support plans that incorporate wellbeing needs, strengths and goals.
* Assist customers to improve independence via skill development, transport, social activities, finances, appointments, cooking, medication and personal care.
* Encourage participation in activities of choice (social, leisure, sporting, educational, employment).
* Keep customer records updated daily using online systems and electronic databases.
* Develop and maintain professional relationships and networks with customers, families, caregivers and key stakeholders.
* Support customers in navigating the sector to access additional services.
* Seek innovative ways to enhance the recovery journey and overall customer experience.
Qualifications
* Minimum Certificate III / Certificate IV in Disability, Community or Mental Health, or willingness to obtain.
* A strong set of values that aligns with passion, integrity & respect.
* Proficient computer skills and experience with Microsoft Office and customer relationship management systems.
Pre‐employment Checks
* Current Police Check (issued within last 3 months).
* Current NSW Driver's License.
* NDIS Worker Check (or willingness to obtain).
* NSW WCC (or willingness to obtain).
