**YOUR NEW ROLE**:
This is an exciting opportunity working in the Short-Term Restorative Care (STRC) Program and providing support for our home and community care clients. As one of our Case Managers, you will play an important role in managing the delivery of the STRC program ensuring services operate to budget. You will be responsible for high quality, person-centred care aimed at supporting older people living at home to maintain their independence and improve their wellbeing.
STRC is an 8-week program involving a multi-disciplinary team who work together to plan and deliver a full range of care and services for the client which aim to reverse or slow functional decline. You will be responsible for assessment, and leading the case management and coordination of in-home therapy, services and equipment. You will ensure the clinical needs of clients are met via goal-directed care plans, as well as maintain excellent relationships with internal and external stakeholders.

**What you need to succeed?**
You will be passionate about providing outstanding service to clients, a natural team player and have:
- Tertiary qualifications in Allied Health or Nursing
- Complex case management experience including working with Clinical or Allied Health professionals
- Experience working in a person-centred, holistic way to achieve outcomes with clients
- Highly developed communication and customer service skills
- The ability to balance competing priorities, teamwork and self-care
- Strong networking skills with key stakeholders
- A solutions-focused approach and ability to link people with a range of suitable support options
- Current drivers licence
