About the role
Liaise effectively at the hospital-community interface to facilitate both primary care and community-based specialist palliative care providers to continue appropriate delivery of care for palliative care patients in the location of their choice, including timely transfer in and out of hospital and the provision of expert advice. Participate in the supervision and training of palliative medicine trainees and other junior staff including resident medical officers and registrars. Actively participate in quality activities including patient safety, clinical audit, research and service planning activities for the Department of Internal Medicine and the broader Metro North Palliative Care Network.
