Healthcare Service Integration Specialist

The role of the Healthcare Service Integration Specialist is to collaborate with a multidisciplinary team of allied health professionals in delivering integrated and holistic care to patients accessing general practitioner clinics within designated areas.

* Demonstrated experience in service expansion across multiple stakeholders, fostering effective collaboration and cooperation.
* Able to initiate, facilitate, and maintain relationships with key stakeholders to drive service integration, leveraging their expertise and knowledge.
* Proven ability to support change management processes, ensuring seamless transitions and minimizing disruption.
* Familiarity with clinical and social issues affecting individuals living with chronic diseases or illnesses.
* Advanced communication skills, including interpersonal, written, and verbal abilities, enabling effective engagement with diverse client groups, including those from linguistically and culturally diverse backgrounds.
* Understanding of social determinants of health and health-promoting principles, informing healthcare strategies and initiatives.
* High-level experience using technology to complete reporting requirements outlined by relevant authorities.
* Alignment with primary care principles and values.

Key Responsibilities:

1. Service Development: Develop and implement service integration strategies, working closely with stakeholders to ensure effective collaboration and cooperation.
2. Relationship Building: Establish and maintain strong relationships with key stakeholders, including allied health professionals, general practitioners, and community organizations.
3. Change Management: Support change management processes, ensuring seamless transitions and minimizing disruption to services.
4. Clinical Expertise: Apply knowledge of clinical and social issues affecting individuals living with chronic diseases or illnesses to inform healthcare strategies and initiatives.
5. Communication: Utilize advanced communication skills to engage effectively with diverse client groups, including those from linguistically and culturally diverse backgrounds.
