**The Role**

The CHOICES Support Coordinator will engage NDIS Participants and their families, who have support coordination in their plans, in the development of their CHOICES Service Agreement and support arrangements to ensure that their needs and wishes are addressed, and NDIS goals achieved. The Support Coordinator will promote individuality, positive outcomes, choice, and control.
You must have a strong belief that all people can achieve outcomes, have value, and contribute to their community. All decisions and actions of the employee will be consistent with the agency mission and values and must be completed with a person centred and customer focus.

**About You**
The ideal person we are looking for has:
- Experience in disability, mental health, and/or community services.
- Strong understanding of and experience with the NDIS.
- Excellent communication and problem-solving skills.
- Ability to work independently with excellent time management skills.
- Collaborative approach with team, clients, and stakeholders.
- Relevant qualifications (e.g., Cert IV in Disability, Social Work, Community Services).
Successful applicants must complete an** employment screening background check**.
You must also hold:
- A current driver's licence, and
- A Working with Vulnerable People Card (ACT)
- NDIS Worker Screening Clearance
