About the Role
As a Social Worker (AHP2), you will deliver high-quality, recovery-oriented care to adults with moderate to severe mental illness in a community setting.
Working across a 7-day roster, you'll balance planned therapeutic work with responsive, urgent care.
Typical weekly activities
* Conducting community-based psychosocial and risk assessments for consumers in acute distress.
* Supporting people to avoid hospital admission where safe and appropriate.
* Developing care plans and providing targeted, goal-directed interventions.
* Working closely with psychiatrists, nurses, and peer practitioners in daily team collaboration.
* Participating in duty/rapid response to ensure timely and coordinated care.
* Engaging families, carers, and external agencies (including NDIS, housing, NGOs).
* Supporting safe transitions and discharge planning.
What You'll Bring
You are a thoughtful, adaptable clinician who works well in dynamic environments and values team-based care.
* Experience in mental health, community, or health-related social work.
* Confidence in psychosocial assessment and care planning.
* Understanding of recovery-oriented and trauma-informed practice.
* Strong communication skills and ability to engage diverse populations.
* Knowledge of (or ability to quickly learn) SA Health systems and the Mental Health Act.
Essential: Qualification eligible for AASW membership.
