Collaborate with medical officers, nurses, allied health and community partners to provide seamless aged care support in a region renowned for its relaxed lifestyle, waterways and welcoming communities.

About the Role

As Nurse Practitioner / Transitional Nurse Practitioner - Aged Care, unleash your advanced clinical expertise to deliver high-level, evidence-based care for older people managing complex conditions such as dementia, falls, cognitive impairment and functional decline. Work autonomously within your endorsed scope, making diagnostic, clinical and prescribing decisions while partnering with geriatricians and multidisciplinary teams to create integrated management plans that enhance safety, quality and care experiences. Champion the Residential Aged Care In Reach (RACI) program to prevent hospital admissions through early intervention, comprehensive assessments and coordinated care, while driving service planning, quality improvement and aged care education across the region.

About You

You're an experienced, autonomous Nurse Practitioner (or transitioning toward endorsement) with proven aged care expertise and a passion for elevating older people\'s health outcomes.

You bring:

* Current AHPRA Authority to Practice as Registered Nurse with Nurse Practitioner endorsement (or substantial progress toward endorsement)
* 3 years' full-time equivalent advanced practice experience in aged care (within past 6 years)
* Eligibility to drive in NSW
* Advanced skills in aged care assessment, diagnosis and management
* Strong communication and mentoring abilities
* Commitment to evidence-based practice and continuous improvement
